there does look to be quite a thickness of sound wood on that ash cylinder, maybe it wouldn't have failed any time soon.

p52 Lonsdale suggests not less than 30-35% stem radius of sound wood, or safety is compromised.

Mind you, if someone had offered me the work I would have gritted my teeth and taken the money.
