it has come to our attention that in an extremely small quantity (0.00005% = 1 out of 2 million) of COBRA ® buckles there may exist parts where the rivets have not been press fixed.

Nice one! Noticed that rivets in general seem to be a common manufacturer's oversight failure point - there was a thing a while back on Treehog spurs, saying to check the rivets were the new type, as the old type were failing. Only time I had a chain break on me was from a shop-made loop where they hadn't spun the rivet - just whacked it.

I checked mine couple of weeks ago and found this...,,,

Seems to have happened as a result of chafing on the lower D's which is surprising as there are no rough edges.

 

I set it up when I first bought it a couple of years ago and never re-adjusted it, ever. Note to self.. readjust the straps on the leg loops periodically to prevent it wearing out prematurely.

 

I know this isn't the same issue highlighted in the OP but if if helps prevent the same happening to anyone else it's worth it.
