That looks like Chalara to me. Those lesions and the reddish brown discolouration are typical.

The effect of the disease is cyclic, with multiple infections causing a cumulative effect over a number of years. Infected areas die back but in the early stages don't have much impact on the overall health of the tree. Over successive seasons the tree will tend to get reinfected, with more and more of the canopy showing symptoms.

The black wilted leaves are only seen on newly affected growth, but I think that what you are seeing is poor growth from a part of the tree previously damaged by Chalara.
