On another note the HSE assume that everyone is incompetent, they have to.

An interesting summation, and probably not far off the mark. HSE will always want to check training / inofmration / instruction etc. and evdience of refresher training is something they're often keen on too.

If that 20 pages of waffle identified the need for inexperienced workers (or idiots) to operate under supervision and was properly implemented, the accident almost certainly would have been prevented.

 

I think it would be better to identify the person as an idiot and keep them away from rotating machinery of any type. Thats why we have brain surgeons, commercial pilots and people who collect shopping trolleys for Tescos.

Indeed.

 

That's part of a safe system of work, and they didn't have one. These things need to be written down so that (a) when something goes wrong you can prove the process has been gone through and (b) it's available to everyone.

 

It's not hard to do and had it been done in this case, Tesco's would have had access to another candidate for shopping trolley collection :001_rolleyes:

Compays fault and there bang too rights plain and simple, they left a 16 year old kid using a defective chipper WTF dont bang on about the kid having no comman sense the co owner seem to have even less!!!!!
