The tying down was inexcusable. The trailer probably wasnt actually overloaded. They are 3.5 ton gross, and is probably plated to carry 2.7 tons.

 

Not all Ivor Williams are 3.5 tonne, some are much less, hard to say from the pic.

Surely, despite what the trailer may legally carry, common sense would kick in here. I mean, you just have to look at the load to know it's wrong. Old bill would have a field day with his ticket book for something like that. I sure as heck wouldn't want to be towing that thing down hill because I reckon you wouldn't be stopping in a hurry if it all went kinda squirrely.

That is surprising!

 

I had a strap come loose and then off on my little hired stump grinder today. It was only in a 6th ifor trailer which has sides on so it just slid left to right a little bit, still a surprise so see it come off even when it's out on tight!

if the dumper was in gear and handbrake on and the one strap tight id say that would be fine. Has anyone ever had a strap work its self loose?

 

Yes especially with flexible materials, that includes tyres bulging in multiple directions. Also only one hook on each side on the trailer, I have seen these bent and sheared off by over tightening, wouldn't give much hope for a shock loading on one. Its this kind of thing we need vosa for. But instead they seem to get those that try to comply.
