2 and 3 man pole rescue are taught at our college and on cs38, several methods from attaching to casualty, bringing them down with you to lowering casualty on their own.

Just seen this and yes, if you pause the vid at 35 secs the lowering rope is running out to another tree or anchor point well away which causes the initial loading as the top gets caught. The line does not look all that heavy and it appears to have been run through a krab not a pulley so the minimum bend radius is well out of the window hence the failure and resultant accident.

Had his rigging line been anchored directly below him at the foot of the tree it would still have failed but with much less of a battering for the climber.
