you have to wonder if either the machine had been LOLER inspected & whether the guy was wearing a harness

Awful news... There are MEWPS Everywhere here in London. I,ve worked off a few tall ones in the past but still prefer hanging in my harness of a nice wooden anchor point!

Terrible for all those involved. You can see how the metal failed in these pics. This type of failure just should not happen on these machines.

Shocking close up pictures! E.S PLATFORMS are a big company. Very true that it Shouldn,t happen.

These things are tested to destruction in every conceivable scenario (as you would damn well expect!). Which suggests that either it was overloaded (unlikely), poorly maintained or had been modified in an unauthorised manner.

 

I'm sure there are a few twitching backsides at ES.

Horrible news, and thoughts are with his family and friends.

 

The harness thing amazes me though. We hired a MEWP from a big company down here (I won't name the company, but it's something that lots of aging actors have), and they don't provide a fall arrest kit when hiring out. I don't know why. I can imagine how someone could assume it's safe as it is hard to fall over the railing, but if something gives like this then there's no chance.

So it failed on a boom connection. Either the bolts went or the metal inside was weak or worn, if it was worn then it would be down to maintenance failure, someone is going to get the cop for it that's for sure.

 

Never liked the damn things anyway. Thoughts with his family.
