Very sad, could some one follow up so we can see what went wrong so we can avoid it in future?

No matter what job we do we always need to be on the ball and just take a moment to think if this went wrong what could I do.

Just had a Facebook update from the apf team terrible news that a member of staff had been involved in an accident whilst setting the show up yesterday and died in hospital last night.
