Log it on Test Cert and Return some inspectors actually put a tag on it as well or if a few items fail then i put them in a box or bag and Clearly label fail as well as listing on test report sheet

I would expect the item to be labelled as failed & returned. As an inspector, I will only destroy or dispose of an item with the owners consent or by prior arrangement, as It is not mine. I will strongly advise it though, in certain circumstances to prevent further use.

If a Rope Fails due to Cuts Etc he has to log it as such he may be polite and Ring Whos kit it is and ask him if he wants it shortening and logging then can be retested and listed as shorter one

It is not the LOLER examiners responsibility to dispose of or quarantine failed items. Only to examine them to determine whether they are safe to use or not. If I fail a rope I will ask if the owner/operator would like it shortened. Hopefully good communication and common sense prevails between examiners and clients.
