Think it largely depends on the insured being aware of exactly what they're insuring and what the policy actually says.

I believe problems arise when someone has renewed a policy several years running without accounting for the extra stuff they accumulate, or the increased cost of replacing items.

 

And of course when someone claims, and discovers they're under insured, they feel the insurance company has shafted them 

 

The onus really is on the insured to make sure the policy is suitable and meets their needs. Renewal time should be looking at everything again to ensure it's suitable, rather than a glance at how much it has increased by.
