PUWER and LOLER inspection templates?

Sorry to be on the blag yet again and I know that I should probably spend 45 quid to the AA but, does anyone have a link to some templates for PUWER fortnightly service/kit inspections or LOLER weekly inspection templates. Hope you can help as I'm only a poor one man band who's trying to get on top of his H&S quagmire!!!

Why dont you draw some up of your own,I did them for previous companies i have worked for.

PUWER:all i put on these was make & model of machine/equipment a check list comprising of all the legal safety features etc,Any faults that you have found & any remedial works required to make fit for purpose.

LOLER:Get a copy of your inspections sheets & copy the equipment down oneside & then have tick boxes for the days of the week,When you have used the kit tick which days you used it & if anything is not right make a comment stating what it is & the remedial work required to make fit for purpose at end of week.

yeah cheers I've sussed out the how to get the table bit on the puter now, better in the tree than the office!!
