never happened to me mate..touch woood!...worst ive come off was felling out 30 ft of black pop tops straight off that grew at 35 % angles and the stems stood up straight when you waved the tops good bye...idea being there was no room for snatching and the tops would role straight out of the sycomores the other side but theyflicked you straight off! found with snatching you atleast end up where you started

On a leaning poplar stem, i was cutting a wedge out so i could bury the dogs of the 288 in, pulled the saw up, spikes kicked out and a 288, tied onto my right side, slid off to the the left.

 

Another time, i was new to rigging, i tried to use a half hitch to rig out a block. two mistekes made, i forgot to use a peg (i was new at the time and i was tired after 6 hours up the tree), and i flicked my scare strap in front of the lowering line. funnily enough the hitch slipped knocked my spikes out and the chunk pulled my lines down, i slid about 15 foot before the rope caught a peg. nothing to do but get back up and take smaller chunks. lesson learnt
