I can see two holes are to tighten both pulling taper in; but my question is how many grub screws should I use i take it to only use the hole with both threads to hold in place but do I leave other grubs in place or one or none?

The taper lock pulleys I had to deal with had 4 holes but the thread  was  in halves, i.e you could use the half thread in a hole to either force the inner from the outer or to  screw the inner into the outer, depending on which pair was used.

Thanks I was thinking i need to use two alright and I think my problem is the inner has lost threads on one hole so I’ll try retap or just buy a new inner

Tapered locks are generally cheap to buy so personally if there's any doubt  , I'd replace it.

I fail to see how a hole can be retapped , I agree with @Jase hutch buy a new inner.
