No idea what this plant is in my garden, can anyone help?

Newly moved into house and this trailing plant in the garden has me baffled.

Does anyone here know what it is? I'm a bit concerned as it looks as though it isn't 100% healthy.

Would like to know what it is so I can find out how to look after it.

It usually looks a bit shabby at this time of year, especially after long periods of dry weather, it’s a consistent Winter flowering plant.

Cut all the flower head stalks back to ground level before they set seed and it usually perks up a bit.
