What's the purpose of the survey? They tend to be in different formats, depending on what the information is going to be used for:

Eg

BS 5837 type that's used for grading trees for retention and identifying constraints to development.

Hazard assessment type - there's a few on this link: http://arbtalk.co.uk/forum/business-management/5341-survey-template-anyone.html

Management type

Part 1 (P1)

As a group, carry out an assignment which will involve surveying and inspecting 40 specified trees on the college estate.

The objectives for the survey will be set during a group discussion involving your tutor.

The required information will be collected by visual examination (VTA).

This information should be presented and submitted as an Excel spreadsheet with an appropriate introduction and an explanation of the information gathered.

Groupwork may be involved in the collection of the information and the formatting of the spreadsheet but the introduction, completed spreadsheet and explanations must be the individual work of each student.

Part 2 (M1)

In relation to the survey carried out for P1 of this unit, describe the techniques that are currently available to detect invasive and semi-invasive decay in trees.

Explain which techniques could be used for the particular trees surveyed and give details of how and when the techniques should be used.

Part 3 (D1)

Produce a survey and inspection programme for the 40 trees used for P1 of this unit. The programme should extend over a five year period and must cover all objectives set for P1.

Your programme should be arranged in chronological order with the most urgent operations taking priority.
