I would consider treating the Scale, mulching the rootplate and doing a light tip reduction to encourage shoots within the crown and aim to get the crown into a tighter more compact form over say three prunes over 15-20 years.

I'd fell it, because that's what the customer wants doing to their tree, the mouldy, root-scuffed, wonky old thing!
