Matheny & Clark inspection checklist

Anyone have a copy of the tree inspection checklist from Matheny and Clarks book, I think it's from the photo guide book. I've done a bit of searching on t'net but can't find it at the moment.

It's Scott's one:thumbup1: but I had forgot about that one. Part of an assignment to create a checklist for surveys, mainly for error proofing the results. I'm looking at what's out there and trying to compile my own. Thanks for the reminder.
