Equipment and vehicle Maintenance Sheets

Hello all,

 

Does anybody have any decent examples of both equipment and vehicle maintenance sheets.

 

I've been trying to create my own with word and tables but can't manage it!!

 

Any help would be greatly appreciated... finding that equipment and chippers etc just being run into the ground with nobody taking any responsibility for looking after their equipment, I feel this might help?
