Whats up with this Sorbus?

Hi All

A friend of mine has this Sorbus that has died and she would like to know the possible cause, any ideas?

It was planted in '95 and has been fine but then failed to come into leaf this spring. I'm not sure on the situation as regards to roots as she has only sent me these photos.
