Stein bollard ratchet handle problem

Has anybody else had this problem? Come down from the tree at ten for a cuppa and to move the bollard to the other side of the tree and as I released the ratchet the handle fell apart! Hadn't knocked it, it wasn't bent. As you can see in the pics the metal pins that get knocked over to stop it pulling apart weren't tapped over enough causing it to pull apart. Ive reassembled it now and burred them over with hammer and chisel to stop them pulling through.
