I need some help please in designing my LOLER record sheet!

Could you give me a little guidence, as at the moment I've decided to sepperate the Climbing / Rigging / Airel rescue equipment and then each crab numbered for each so Climbing C1 Rigging R1 etc. I'd like to keep it a5 size but the ones I've reserched all seem to be A4, is this an issue.

I look forward to your advise.

Thank you Timon,
SWL - safe working load. is that a necessety or applicable?

With rigging equipment yes. Just mark the column ‘PPE one person’ for climbing items.
Or ‘PPE two persons’. For rescue kit.
